Once you have a reservation, be sure to bring:

• Social Security card or ITIN for each family member
• W-2 forms for all jobs held in 2015 and all 1099 or 1098 forms
• Child care provider information
• A voided check to direct deposit your refund
• Last year's tax return (if you have it)
• Health insurance statement form 1095-A, 1095-B, or 1095-C (if you have it)
